What We Do Our Services & Programs We have been the sole provider of Care Coordination (case management) and Recovery Support Services in Prince Georges County since 2012. We have effectively case managed, supported, and assisted over 4,500 Prince Georges County residents.

State Care Coordination (SCC) services are mandated treatment recovery support services for Prince George’s County residents funded by the Prince George’s County Department of Health and the Local Behavioral Health Authority.

The purpose of SCC is to improve recovery outcomes for individuals identified as high risk for relapse and ensure the removal of recognized barriers to recovery. Such obstacles can include unstable housing, lack of vital documents, transportation, social service benefits, education, unemployment, family & mental health/health problems, etc.

SCC services are designed to provide case management and recovery support assistance by walking with individuals through the recovery process; and providing referrals and linkages to county, community, faith-based, and other human services organizations. SCC services overall goal is to create a more significant opportunity for a client to be successful on their journey to remaining drug or alcohol free.

Case Management & Referral Services

DPP conducts initial intakes, confidential needs assessments; and prepare a service plan with the appropriate support services to meet the needs of the individual; as well as providing assistance by linking them to available substance abuse, mental health,medical, human services, employment; other resources & opportunities to meet their specific needs.

Vital Documents Assistance

DPP assists eligible program participants with obtaining vital documents such as State-issued Identifications and Birth Certificates to eliminate barriers to receiving social services benefits and employment.

Transportation Assistance

DPP seeks to assists eligible program participants with Limited Transportation Assistance. Our goal is to ease the stress; and provide easier access to necessary medical, social services, program activities, educational and employment opportunities.
